Accident Reporting
In case of an any accident that requires medical treatment, the first priority is the well being of the injured person, especially if that person is a player.
If the injury is serious, call 911.
If the injury is less serious but still needs treatment and the if injured person is a child whose parents are present, they may consent to transporting the child to the doctor themselves.
If the injured person is a registered player or volunteer (such as a Coach or Assistant-Coach), then certain information must be filed with the National AYSO office and the Regional Safety Directory. Be sure to note the date and time of the accident and how it happened. Collect the names, addresses and phone numbers of other adults who was present.
Forms
Two forms are required, both of these are available on-line in Adobe PDF format. Both have active form fields that can be filled out on-line before printing. After bringing up the form in Acrobat Reader, click on a field. If it can be filled in on line, you will get a vertical bar prompt. After filling in the forms, print two or three copies.
- Accident Report Form (will open in new window): Print out, fill-in and make two copies. Send one copy to the Safety Director c/o the Region's mail box. Also notify the Safety Director by email.
- Participation Release Form (will open in new window): If medical treatment was required by a player, then before that player can practice or play in a game, you must obtain a participation release form signed by the treating Doctor and a parent. Keep a copy and send one to the Safety Director
AYSO National provides supplemental accident insurance to all registered players and volunteers. The insurance claim form is available to those who wish to make a claim. This insurance is secondary to the injured person's own insurance (if they have any) and includes a $200 deductible.
